Why the Calories You Think You're Eating Aren't the Calories You're Actually Absorbing

This is part 2 of a series on why weight loss stalls even when you're doing everything "right." In part 1, we covered the biology of perimenopause and menopause, why the scale sometimes lies, what estrogen and testosterone actually do to your metabolism, and why your thyroid test might not be telling you the whole story. You've started logging your food. You're aiming to become more aware of what you're actually eating. The effort is there. But the scale isn't moving the way you think it should-and you’re frustrated. 


Before you assume something's broken with your metabolism or all of the effort just isn’t worth it, let's dig into the five aspects of food tracking and metabolism that most people never learn.


There Are Actually 5 Different "Calorie Numbers"


When most people think about calories, they think there's one number. There isn't. There are five:

  1. Actual calories eaten (what goes down the hatch!)

  2. Recorded calories (what you write down or log)

  3. Listed calories (what the label says)

  4. Metabolizable calories (what your body actually absorbs after digestion)

  5. Retained energy (what actually gets stored, after everything else)


Here's the part that matters most: when you track your food, you're usually only capturing the recorded and listed calories, and even with that, there is a lot of room for error. So much of the story lives in the other three aspects, and we’re going to dig into it all today!


Tracking Inaccuracies: You're Not Doing It Wrong on Purpose, but You Probably Aren’t Doing It Accurately Either. 


In a well known study, researchers brought a group of dieters who reported eating 1,200 calories a day and could not lose weight into a metabolic ward. Using doubly labeled water, the actual gold standard for measuring intake, they measured exactly what these people were eating and exactly how much they were moving.


The results are nuts. These dieters were underreporting their actual food intake by an average of 47%. At the same time, they were overreporting their exercise by 51%. 


That's not a small rounding error. That's the difference between "I'm eating 1,200 calories" and eating closer to 2,400 calories, while also believing they’re moving a lot more than they actually are.


Their metabolism wasn't broken. They weren’t intentionally lying. Their perception of their own intake was simply way off. This is what genuinely happens when humans estimate their own food and movement without training the skill of accurately measuring & tracking.

It's not just people who struggle with tracking either. Separate research using simple food recall found people's estimated intake ran 15-20% below what they actually consumed, which can add up to an additional 400-600 calories a day. Even registered dietitians, professionals trained to do this exact thing, underreported their own intake by 223 calories a day on a 7-day food log, versus over 400 calories off in the general population. 


Translation: if a client swears they're eating 1,200 calories and they’re not losing weight, the food log is almost never lying on purpose. It's just genuinely hard to track with precision, even for the experts. This is exactly why tracking accuracy is a skill worth building deliberately, not something to assume you're already doing perfectly. It’s also important to understand that what your app tells you is likely not actually what you’re consuming every day. 


Label Accuracy: The Number on the Menu Isn't the Number in the Kitchen

The FDA allows restaurants and labels up to a 20% margin of error on listed calories. In practice, restaurant meals tend to run over that 20% far more often than under it, and restaurant meals already tend to be much higher in calories than their home cooked counterpart. 

None of this means labels are useless. It means a little cushion for error is baked into the system before you've eaten a single bite. That turns a 1200 calorie day that was IMMACULATELY tracked into a 1,440 calorie day.

Metabolizable Calories: The Food Matrix Effect

Here's where it gets genuinely interesting. Two foods can have identical listed calories, and your body can absorb completely different amounts of energy from them, depending on their physical form.


Take whole grains vs. refined grains. In one controlled 6-week study of adults ages 40-65, researchers swapped nothing but the grain source, whole grains for refined grains, while keeping calories, macros, and meal structure identical between groups. The whole grain group lost 57 more calories through stool and their resting metabolic rate increased by 43 calories a day. Combined, that's roughly 100 extra calories a day, from a single swap, in people the same age as most of your clients. 


The foods you choose matter. Whole, less-processed forms of food often means fewer calories actually get absorbed, even when the label may show the same number. 


Your Gut Bacteria Are Deciding How Many Calories You Absorb

Your microbiome isn't just along for the ride, it actively determines how much energy you extract from what you eat.


In one study, participants ate a "microbiome enhancer" diet, essentially a Mediterranean-style pattern with whole, less-processed foods, more resistant starch (like cooked-and-cooled potatoes or rice), and minimal ultra-processed foods. Compared to a typical Western diet, this group lost about 116 more calories a day through stool with some participants losing close to 400 calories daily! On average, only about 89% of listed calories were actually metabolizable. That sounds like a pretty great trade for me if we want to lose weight while eating more food!

Okay, That’s Great Erin...But What Can We Actually Do With This Information?

  • First, we can stop assuming a stalled scale when weight loss is a goal means our metabolism is broken. Sometimes calories may look really low when in fact, we’re consuming much more food. 

  • Tracking is a skill that can be developed and honed and it’s a worth while pursuit if you’re trying to lose weight or improve your body composition. Don’t expect perfection with yourself, understand that trained professionals get it wrong by hundreds of calories. The good news is that it's a skill you can build, refine, and get better at!  It will get easier and you will become more accurate the more you practice it with the right guidance.

  • Let’s stop treating all calories as identical just because a label says so. Whole, minimally processed foods tend to deliver fewer usable calories than their processed counterparts, even at the same listed number. 100 calories of a baked potato and 100 calories of potato chips are not the same. 

  • Start paying attention to gut health as a genuine lever, not just a digestion issue. It directly affects how many calories someone actually absorbs from identical food.
