Why Can't I Lose Weight in Perimenopause? The Real Science Behind It

If the scale is climbing and your clothes fit different in ways that don't add up, you're not imagining it. Your body composition is shifting faster than your body weight is, and there's real research behind why.


Let's break down what's actually happening, backed by scientific research: 



The Study of Women's Health Across the Nation (SWAN) followed over 1,200 women through the menopause transition and found something important: roughly two years before your final menstrual period, the rate of fat gain approximately doubles. Lean mass declines at that same accelerated rate. That sounds like zero fun to me. 


Here's the part that messes with people's heads: whatever the scale is telling you, it's underselling the real story. The lead researcher on this study put it well: you can accumulate more fat without the scale reflecting the true shift, because you're simultaneously losing lean mass. Climbing slowly or holding steady, the number doesn't capture what's actually happening to your body composition.


Translation: if you're only tracking the number on the scale, you're missing the actual story. This is exactly why we use tape measurements and body comp tools like DEXA when we can instead of relying on scale weight alone.

If you're in your 40s and not yet close to this 2 year window, this still applies to you. In our 40s, hormones are already getting a bit chaotic, sleep quality often starts to suffer, and if we aren't actively strength training to build or maintain muscle mass, we're slowly losing it in the background. All of that can start changing your body composition well before the SWAN study's two-year mark. This isn't a switch that flips overnight. It's a slow build, and the earlier you support it, the better. 


In all honesty, when women come to me, they aren’t coming to me because only their body composition has changed, they are coming to me because they’ve gained weight, they have more midsection fat they never had before, and they don’t feel good about it. 


Estrogen Isn't Just a "Hormone Thing." It's a Metabolic Rate Thing.

We all know estrogen declines in this stage of life. As it declines, energy expenditure declines with it. Research shows estrogen HRT has been shown to raise resting energy expenditure by as much as 222 calories a day!


Without HRT, resting metabolic rate can decline by roughly 100 or more calories daily in menopause. This is largely from the loss of lean mass. The important thing to understand here is that HRT doesn't hand you free bonus calories on top of your normal metabolism, it helps offset a decline that's already happening. Still, that's not a small number to have working against you (or for you) in the background. That could theoretically add up to something like 10+ lbs over a year if nothing else changes. This makes so much sense as to why women say “Nothing has changed and I am just gaining weight!” It’s a scary place to be, but I promise you things are changing underneath the surface, and when we know better, we can do better.

Testosterone Replacement: Not for Everyone, but Worth Understanding

Testosterone replacement isn't right for every woman, and if you're not a good candidate or you're not interested, that's completely fine. It's often misunderstood and plenty of women feel nervous about it, which is understandable based on what is commonly known about this very important hormone for women. 


For women who are good candidates and choose to undergo testosterone replacement in an intelligent way (and that part is important!), lean mass tends to increase. If you're only watching the scale, this can feel discouraging or even like failure, since muscle takes up less space than fat but doesn't disappear from the number you see.


This is why body comp tracking matters so much here. A woman gaining muscle and losing fat at the same time might see the scale barely budge and assume nothing is working, when in reality, everything is working.


Insulin Resistance and Leptin Resistance: The Quick and Dirty Version


These terms get thrown around a lot, so here's the plain-language version.


Insulin resistance means your cells stop responding well to insulin, the hormone that helps move sugar out of your blood and into your cells for energy. Your body compensates by producing more insulin, which over time makes it harder to burn fat and easier to store it, especially around the midsection.


Leptin resistance works similarly, but with the hormone that tells your brain you're full. When your brain stops "hearing" leptin properly, you can feel hungry even when you have plenty of stored energy to draw from.


These two frequently show up together, often alongside low-grade inflammation, and the symptoms overlap enough that they're usually addressed with similar strategies: blood sugar stability, strength training, sleep, and stress management.


Thyroid: The Master Regulator of Your Metabolism

Your thyroid is the master regulator of your metabolism. It influences how you make and use energy at a cellular level, which means when it's off, everything downstream can feel harder: energy, weight, mood, digestion, temperature regulation, all of it.


Here's the problem. When most Western medicine providers "check" your thyroid, they usually only pull TSH. TSH is one piece of a much, much bigger picture, and it doesn't tell you the whole story of how your thyroid is actually functioning. A full picture requires a broader panel and someone who knows how to read it in context with your symptoms, not just a single number that falls in a wide "normal" range.

Sleep and Hormones: A Two-Way Street

Perimenopause and menopause come with their own wonderful gifts of sleep disruption, night sweats, racing thoughts at 3am, and waking up wired for no clear reason. This isn't just annoying. It has real metabolic consequences.


Poor sleep during this season can lower your metabolism, further disrupt an already shifting hormone picture, and directly affect your energy levels and how you feel day to day. It becomes a loop: hormones disrupt sleep, poor sleep disrupts hormones further, and the whole system feels harder to manage. This is exactly why sleep isn't a "nice to have" in a plan built for this stage of life. It's foundational. 


Inflammation: The Hidden Piece Most People Never Test For

Low-grade inflammation is sneaky. Most people have no idea it's there because it doesn't show up as an obvious symptom, it shows up as "I just don't feel like myself" or a stubborn stall that doesn't make sense given everything you're doing right.


Often, it's built quietly through everyday lifestyle factors: chronic stress, poor sleep, blood sugar swings, gut health, poor diet, too much alcohol, and more. The only way to actually know if your body is inflamed is to get labs and take an honest look at your lifestyle alongside them. Once that inflammation is addressed, some clients see meaningful shifts in their weight without changing much else.


So What Do You Actually Do With This?

You stop measuring your progress with a single number and start measuring the whole picture: body composition (this matters more than just how you look in the mirror) energy levels, sleep quality, strength, and yes, the scale too, just not alone.


You stop assuming stalled progress means you're doing something wrong, and start asking which of these systems might need support: hormones, sleep, insulin, thyroid, inflammation.

This is precisely why generic "eat less, move more" advice fails so many women in this stage of life. Your biology changed. Your strategy needs to change with it.
